Abstract

 

Post-secondary transition for individuals with disabilities is a global concern. This article investigates several related issues, namely: global commonalities of post-secondary transitions; importance of transition planning; developmental considerations of individuals with disabilities; and best practices for post-secondary transition. The authors also present the Summary of Performance model for Individual Transition Planning (ITP) documentation between the secondary school and the post-secondary educational setting.
